Clutch Fork Broke...How?

Clutch Fork Broke...How?
The other day driving home the clutch and transmission decided to stop working suddenly. Everything was great before hand with no signs of trouble. The shop says that my clutch fork broke....how does that break?? Being an engineer, that is one part of the car that should (and probably is) designed to outlive the entire car itself and never break.
Since they have to replace the fork, they are doing the master/slave cylinder since it was beginning to leak and I am doing the clutch also since the trans is out. Car only has 73K miles on it and is a 97 Formula M6. No idea how or why that part would break.

The fork has a little clip on the back of it that holds itself up when on the throw out bearing. I broke that off and there is no way to fix it but replace the fork. Fork itself was fine.
