Didn't the code for the MAF disappear?

There is no way a problem with the MAF will set the codes for the Opti optical sensor.

Considering that it appears to shut down once it goes into closed loop. And it had a code for O2 switching problem. I would unplug one of the O2's and see if it runs better by forcing it to stay in open loop.

If the MAF were the problem, simply unplugging it should show the problem since that will force the PCM into speed/density mode. Assuming of course the MAP is providing a good signal.

So it would appear the MAF isn't the problem. Now try unplugging an O2

I replaced the one 02 sensor that was showing up on code reader.
I also have a exhaust leak in the manifold. Could this cause the o2 sensor to throw a trouble code?
thanks for the help

A leak at the exhaust manifold will cause that bank of the engine to run rich. But unless its an extreme leak, its not going to set a "lean" code.

I would say its a small leak. What should happen when I unplug o2 sensor?

I already answered that question.
