Make sure you have no gas in the fuel regulator vacuum line. Some adjustable regulators are known to have a leaky diaphragm causing fuel to get into crankcase.

The problems you have sound just like the opti... I never had a code, it was new OEM and bad from the factory... It started and ran fine for 10+ min then it would die and be very hard to start..... I checked the voltage on shoebox page it all checked, after replacing tons of other ignition components I finally bit the bullet and warrantee the opti and it was fixed.... I also had a similar problem with Aermotive FPR! I'm firm believer that OEM parts are the best way to go on ignition..

The vacuum line pulls the vacuum from the intake manifold not the crankcase.

I live in Perrysburg and have a 94 Z28. I have been using a laptop and software to log data on my car for over a year now. If you want to log some data on your car to see what it is doing PM me. You are about an hour and 15 minutes from me. You would have to bring the car down here to use everything.

I can also tell you where I got everything to hook it up to the car if you want to go that route. I think I have about $40 into everything including being able to program the car. I also installed a parallel port in the car so I could just put it in without any adaptors.

The liquid fuel from the FPR vacuum connection enters the intake manifold near #6 and #8. The fuel ends up pouring into the cylinders, washes down the cylinder walls, gets past the rings and ends up in the oil.

Do you think the fuel is going to stay inside the intake?

[edit]Injuneer already explained it well.[/edit]

How Hot?

Guys,
I know where to come when I have an issue now! All of these responses are good. How "hot" is the car getting when it "warms up"? If it is anything like my 97 used to be the gauges all lied and the same symptoms (most of them) that you are describing were related to my engine getting too hot. Which ultimately led to it shutting down and not restarting for the night.
Prior to me, someone had filled the radiator with Bars Leak or something similar and that reeked havoc on everything....you all know where all the coolant is supposed to go....everything with a diameter less than 1/2 inch was plugged.
Car ran great until hot and then backfired, missed and shut down. That is probably not your issue, but I would not rule it out (since you have run through all the harder items already).
The Opti is probably the biggest isue and the EGR block off needs to be done right, if all are OK then retune it with one of these guys pulling the codes.

PS - the car is now a 396 Stroker, but the motor is not dropped in yet so look for some posts from me in about a month!!
