What could have Cause this sort of Damage to Rotor and Pad

Question What could have Cause this sort of Damage to Rotor and Pad

Other day I heard metal grinding sound comming off rear of driver side
Saw WORN off DISC and Used Pad.
I have replace the Disc and pad on both rears.
I notice driver side rear calipar was jammed and slidind rod was rusted Orange
I had to hammer out to undo the sliding bracket so I can re grease it.
However I notice some thing odd.
Driver side rear where bracket was worn out.
Passenger side had half of its life there. I have added following Pics
Can some body what could have cause all that ?

Sounds like a stuck guide pin. You should replace it (if possible) or replace the whole caliper.

Yea that bone Dry and Rusted Orange.
I just relube it with Brake synthetic lithium grease and put it togather
Should I be worried

Keep an eye on it. It will do the same thing to the new pads, if not corrected.
